Deficiencies

1) Title 9, Section 10584(b) Fixtures, Furniture, Equipment and Supplies Class C

The above section states, “All window screens shall be in good repair and free of insects, dirt and other debris.”

The licensee was deficient in meeting this regulation because the bathroom’s window screen has a gaping hole.

2) Title 9, Section 10584(e) Fixtures, Furniture, Equipment and Supplies Class C

The above section states, “All toilets, hand washing and bathing facilities shall be maintained in safe and sanitary operating conditions.”

The licensee was deficient in meeting this regulation because the resident’s bathroom located in room #203, #208, #210, and #211 are not clean or sanitary and the showers have mold.

3) Title 9, Section 10584(e) Fixtures, Furniture, Equipment and Supplies Class C

The above section states, “All toilets, hand washing and bathing facilities shall be maintained in safe and sanitary operating conditions.”

The licensee was deficient in meeting this regulation because the tank cover does not fit properly in the bathroom located in room #203.

Deficiencies 2

1) Section 12070(a)(1-4) Recovery or Treatment Planning

The above section states, in part, “The program shall provide services to ensure that all program participants develop recovery or treatment plans. The recovery or treatment plan shall include, statement of problem experienced by participant, statement of objectives to be reached that address each problem, action steps that will be taken by program and/or participant to accomplish the identified objectives, and target date for accomplishment of action steps and objectives.”

• The provider was deficient in meeting the above standards because there was no documentation that the program developed treatment plans for participants upon intake.

2) Section 12070(b)(4)(A)(2) Recovery or Treatment Planning

The above section states, in part, “Staff shall develop the initial treatment plan with input from the participant in accordance with the time frame. For long-term residential programs (program duration 31 days or more) the treatment plan shall be developed within 14 days from the date of the participant’s admission.”

The provider was deficient in meeting the above standard because the program to develop the initial treatment plans within 14 days from the date of the participant’s admission.

3) Section 19005(b)(2) Personnel Policies

The above section states, in part, “The program shall maintain personnel files on all employees. Each personnel file shall contain: Employment confirmation statement.”

The provider was deficient in meeting the above standard because there was no documentation of employment confirmation statements for employees #2 and #3.
