Deficiencies

1) Title 9, Section 10564(e)(1) Personnel Requirements Class B

The above section states, In part, “Good health shall be verified by a health screening, including a test for tuberculosis, performed under licensed medical supervision not more than sixty (60) days prior to seven (7) days after employment ”

The licensee was deficient in meeting above regulation because of the following:
• Employee number and three were missing a health screening report and an updated tuberculosis test results in their personnel file.

2) Title 9, Section 10572(9) Health-Related Services Class C

The above section states, in part, prescription medications which are not removed by the resident upon termination of services shall be destroyed by the facility administrator, or designated substitute.”

The licensee was deficient in meeting above regulation because of the following:
• There was no “Medication Destruction”policy or procedures for medications left at facility after resident termination of services; and
• PRN Medications were not noted on the Centrally Stored Medication Logs

3) Title 9, Section 10573(a)(15) Food Service Class B

The above section, states, “Pesticides and other similar toxic substances shall not be stored in food storeroom, kitchen areas, food preparation areas, or areas where kitchen equipment or utensils are stored.”

The licensee was deficient in meeting above regulation because toxic cleaning supplies where stored in the kitchen area.

4) Title 9, Section 10681(a) Buildings and Grounds Class C

The above section states, “Facilities shall be clean, safe, and sanitary and in good repair at all times for the safety and well being of resident’s employees and visitors.”

The licensee was deficient in meeting this regulation because the following were noted during the facility walk through:
• Back area had old appliances, old car, furniture, and debris.

Deficiencies 2

1) Section 12070(b)(4)(C) Participant Files

The above section states, in part, “Staff and the participant shall review and update the treatment plan when a change in problem Identification or focus of recovery or treatment occurs, no later than 90 days after signing the initial treatment plan.”

The provider was deficient in meeting above standard because participant did not have a 90 day update to their initial treatment plan in the file.

2) Section 19015(a)(1) Health Screening and Tuberculosis Requirements

The above section states, in part, “All staff whose functions require or necessitate contact with participants shall complete a health screening report or health questionnaire.”

The provider was deficient in meeting the above standard because employee number one and three was missing a health screening report in their personnel file.

3) 19015(b)(1) Health Screening and Tuberculosis Requirements

The above section states, in part, “All staff and volunteers whose functions require or necessitate contact with participants or food preparation shall be tested for tuberculosis. The tuberculosis tests shall be conducted under licensed medical supervsion not more than three months and renewed annually from date of last tuberculosis test.”

The provider was deficient in meeting above standard because employee number a current tuberculosis test results in the personnel file.

4) Section 26010(a) Health and Safety

The above section states, “Programs shall be clean, safe,sanitary and in good repair at all times for the safety and well being of participant’s employees and visitors.”

The provider was deficient in meeting above standard because the following was noted:
• Back area had old appliances, old car, furniture, and debris.
