Deficiencies

1) Section 12070(b)(4)(A)(2) Personnel Policies

The above section states, “The program shall provide services to ensure that all programs participants develop recovery or treatment plans. (b) The process for recovery or treatment plans shall be the following:

* If a treatment plan is being developed: (A) Staff shall develop the initial treatment plan with input from the participant in accordance with the time frame below: (2) For long-term residential programs (program duration 31 days or more) the treatment plan shall be developed within 14 days from the date of the participant’s admission.”

The provider was deficient in meeting the above standard because a treatment plan was not developed for client 6 within the time frame allocated.

2) Section 19005(b)(5} Personnel Policies

The above section states, in part, “The program shall maintain personnel files for all employees. Each personnel file shall contain: (5) Employee evaluations.”

The provider was deficient in meeting the above standard because employee from employees 2 and 5.

The above section states, “Programs shall be clean, safe, sanitary and in good repaid at all times for the well being of participants, employees and visitors.”

The provider was deficient in meeting the above standard because of the following:
• Dirty floor in operation manager’s office
• Faucet not working in downstairs women’s bathroom
• Dirty floor in clinical office
• Dirty floor in counselor office 1
• Large chips of paint missing on wall columns of multi-purpose room
• Low water pressure in staff breakroom bathroom
• Laundry machine making loud noises in laundry room
• Hallway lights and bathroom light not working in 2nd floor hallway
• Food items left uncontained in room 209
• Food items left uncontained in room 208
• Pile of dirty dishes in sink of room 208
• Missing mattress pad in room 206
• Uncontained food items in refrigerator of room 205
• Pile of dirty dishes in sink of room 205
• Missing mattress pad in room 204
• Slipping hazard next to bathtub in 2nd floor east bathroom

The above section states, “The licensee, whether an individual or other entity, is accountable for the general supervision of the licensed facility, and for the establishment of policies concerning its operation.”

The licensee was deficient in meeting this regulation because of the following:
• One medication was expired
• Prescription numbers for medications are not being properly documented
• Medication prescription was over counted when reviewed
• Two medication prescription was under counted when reviewed

3) Title 9, Section 10584(9) Fixtures, Furniture, Equipment and Supplies Class C

The above section states, “The licensee shall provide clean linen in good repair, including lightweight, warm blankets, top and bottom bed sheets, pillow cases, mattress pads, bath towels, and wash cloths. The quantity of linen provided shall permit changing, at a minimum, once a week or with greater frequency if needed. ”
