Deficiencies

1) Section 12030 Communicable Diseases

The above section states, in part, “Programs shall have a policy that requires participants who show any signs of any communicable disease to be cleared.”

The provider was deficient in meeting the above standard because the program manual did not contain documentation of a Communicable Diseases policy.

2) Section 12050 Referral For Medical or Psychiatric Evaluation and Emergency Services

The above section slates, in part, “The program shall have written procedures for obtaining medical or psychiatric evaluation and emergency services.”

The provider was deficient in meeting the above standard because the program did not contain documentation of policies and procedures for obtaining medical or psychiatric evaluation and emergency services available for review.

3) Section 12055 Referral Arrangements

The above section states, in part, “If during the course of recovery or treatment services and determined to be in need of additional services, the program shall provide a referral to the appropriate services.

The provider was deficient in meeting the above standards because the program did not contain documentation of policies and procedures that identify the conditions under which referrals are made.

4) Section 12060 Alcohol and/or Drug Free Environment

The above section states, in part, “Recognizing that relapses can be part of the recovery or treatment process, a program shall have written policies regarding service deliver after a relapse episode.

The provider was deficient in meeting the above standards because the program did not contain documentation of policies and procedures for services after a relapse episode.

5) Section 12070(a) Recovery or Treatment Planning

The above section states, in part, “The program shall provide services to ensure,that all program participants develop recovery or treatment plans.”

The provider was deficient in meeting the above standard because various Initials and Updated Treatment Plan_s target dates did not contain documentation of completion date(s) to help clients identify when goals of action steps and objectives were completed.

Suggested: For treatment to be successful it helps to show clients their accomplished dated of goals completed during each treatment plan update or when there’s a change in treatment.

6} Section 12070 (b}(3)(A)(3} Recovery or Treatment Planning

The above section states, in part, “The policies and procedures manual shall contain but not be limited to policies and procedures.”

The provider was deficient In meeting the above standard because there were no policy and procedures on the required time frame for documenting progress and developing treatment plans and/or updated treatment plans for participants.

7) Section 12080 Continuing Recovery or Treatment Exit Plan

The above section states, in part, “Program staff shall meet with each participant to develop and document an individualized strategy that will assist the participant in maintaining a continued alcohol and drug free lifestyle.”

The provider was deficient in meeting the above standard because the program manual did not contain a policy and procedure that identified the conclusion to develop and document an individualized strategy that will assist the participant in maintaining a continued alcohol and drug free lifestyle.

8) Section 12085(b) Discharge Summary

The above section states, in part, “Each program shall have written procedures regarding participant discharge. These procedures shall contain the following.”

The provider was deficient in meeting the above standard because the program did not document the clients discharge summary that shall include:

1. Description of treatment episode or recovery services;
2. Current alcohol and/or other drug usage;
3. Vocational and educational achievements;
4. Legal status;
5. Reason for discharge and whether the discharge was involuntary or successful completion;
6. Participant’s continuing recovery or treatment exit plan;
7. Transfers and referrals; and
8. Participant’s comments missing documentation of participant comments.

9) Section 19005(b)(1-7): Personnel Policies

The above section states, in part, “The program shall maintain personnel files for all employees. Each personnel file shall contain:

* Application for employment and resume
* Employment confirmation statement
* Job description
* Employee evaluations

The provider was deficient in meeting the above standard because the following items were missing from the employee’s file:

(1) Missing updated evaluations for all staff.

10) Section 24000 Confidentiality

The above section states, in part, “confidentiality of participants, participant’s files and information is assured.”

The provider was deficient in meeting the above standard because the program did not have a written statement regarding confidentiality when answering the telephone.

11) Section 26010 Health and Safety

The above section states, in part, “Programs shall be clean, safe and sanitary.”

The provider was deficient in meeting the above standard because the program had damage in the following area:

• The paint was chipping and peeling throughout the facility.
• The bathroom’s (men’s, women’s, and staff/UA facility) need deep cleaning
• The men’s bathroom wall in the stall had holes and the urinals dividers were rusted and corroded
• The floors had debris throughout various areas in the facility.
• The light fixtures in the men’s bathroom were missing covers.
• The ceiling above the stage was missing and exposing insulation.
• The backdrop (100lbs.-2001bs.) hanging above the stage was hung by old, worn out and dusty ropes.
• The window cracked/broken in the UA/staff bathroom on the stage.
• The green chairs in the auditorium were torn and damaged.
• The wooden hallway floor In front of the men’s and women’s bathroom (leading to the big group room) was damaged with holes.

12) Section 26010(a}(4) Health and Safety

The above section states, in part, “The program shall have a written policy possessing guns, knives, or other weapons.”

The provider was deficient in meeting the above standard because the program did not have a written policy that prohibits individuals from possessing guns, knives (other than kitchen utensils), or other weapons at the program.
