Deficiencies

1) Section 12060 Alcohol and/or Drug Free Environment

The above section states, in part, “Recognizing that relapses can be part of the recovery or treatment process, a program shall have written policies regarding service delivery after a relapse episode. Thesepolicies shall be enforced and t;e supportive of and consistent with the alcohol and drug free environment of the program.”

The provider was deficient in meeting the above standard because there was no policy or procedure regarding service delivery after a relapse episode.

2) Section 12070{b)(3)(A)(C) Recovery or Treatment Planning

The above section states, in part, “A. The participant shall develop the initial recovery plan with guidance from staff in accordance with the time frame specified below: B. Staff shall ensure and document that the participant reviews and revises, as necessary, the recovery plan when a change in problem identification or focus of treatment occurs, or no later than 90 days after signing the initial recovery plan and no later than every 90 days thereafter, whichever comes first.”

The provider was deficient in meeting the above standard because there were no clear dates identifying initial treatment plan and no evidence that treatment plans were being reviewed by participants within a 90-day period.

3) Section 19005(b)(6) Personnel Policies

The above section states, in part, “b. The program shall maintain personnel files on all employees. Each personnel file shall contain: Health records including a health screening report or health questionnaire and tuberculosis test results as required.”

The provider was deficient in meeting the above standard because Employee tuberculosis test in their personnel file.

4) Section 26020 Hours of Operation

The above section states, “Each program shall post the hours of operation to inform the general public and participants. When not open, the program shall provide information concerning the availability of short-term emergency counseling or referral services, including, but not limited to, emergency telephone services.”

The provider was deficient in meeting the above standard because there was no emergency telephone services or other referral service information posted outside of the facility.
