Deficiencies

1) Title 9, Section 10573(a)(22) Food Service Class C

The above section states, in part, “Tableware and tables, dishes, and utensils shall be provided in the quantity necessary to serve the residents.”

The licensee was deficient in meeting above regulation because there was not sufficient tableware, tables, dishes, and utensils to serve all residents.

2) Title 9, Section 10581(a) Buildings and Grounds Class C

The above section states, in part, “Program will be clean, safe, sanitary and in good repair at all times for the safety and well-being of participants, employees and visitors.”

The provider was deficient in meeting above standard because of the following:

(1) Library- chipped paint on the wall by Discovery board.
(2) Library- baseboards are peeling away from the wall.
(3) Kitchen-ants on counter tops near the serving window.
(4) Kitchen-two drawers missing.
(5) Kitchen-chipped paint on the walls.
(6) Kitchen-food on the ceiling.
(7) Grand hall- broken blinds on the glass doors opening to the outside.
(8) Bedroom #1- broken.blinds.
(9) Leadership bedroom-chipped paint and a hole in the wall above the bunk next to the door.
(10) Bedroom #3- missing outlet cover over the single bed.
(11) Bedroom #3- the air conditioner cover is missing and the filter is dirty.
(12) Hallway light on the second floor has a burnt out bulb.
(13) Resident lounge- broken blinds.
(14) 3rd floor hallway- hole in one of the screens near the workout room.
(15) Bedroom #4- missing an air conditioner cover and the filter is dirty.
(16) Bedroom #5- missing an air conditioner cover and the filter is dirty.

3) Section 19005(b)(2) Personnel Policies

The above section states, in part, “The Program will maintain personnel files on all employees. Each personnel file will contain employee evaluations.”

The provider was deficient in meeting the above standard because staff #1 and #11 are missing personnel evaluations from their file.

4) Section 19020(a) Staff Training

The above section states, in part, “The Program will foster and encourage the continuing development of staff expertise and staff attendance at appropriate training program. The Program will have a written plan that is annually updated, for the training needs of staff.”

The provider was deficient in meeting above standard because there was no annual staff training plan.

5) Section 24000 Confidentiality

The above section states, in part, “Program will assure confidentiality of the participant and the participant’s files and information in accordance with Title 42, Code of Federal Regulations, Part 2 and, when state funds are used, Health and Safety Code, Sections 11812(c) and 11977. A copy of the federal regulations will be available at each Program. A written statement regarding confidentiality when answering the telephone and confidentiality regarding files will be included in the-Program’s operations manual. Participant files will be accessible only to authorized personnel.”

The provider was deficient in meeting above standard because there was no written statement regarding confidentiality when answering the telephone in the program’s operation manual.
