1) Section 17010 Program Policies
The above section states, in part “All program policies and procedures shall be contained in an operation manual that is located at each certified site and that shall be available to staff and volunteers. The policies and procedures manual shall contain but not be limited to the following: 5. Alumni involvement; 7. Recreational activities.”
The provider was deficient in meeting this standard because the aforementioned items were not contained in the policies and procedures manual.
2) Section 19015(b}(1) Health Screening and Tuberculosis Requirements
The above section states, “All staff and volunteers whose functions require or necessitate contact with participants or food preparation shall be tested for tuberculosis. The tuberculosis test shall be conducted under licensed medical supervision not more than three months prior to or seven days after employment and renewed annually from the date of the last tuberculosis test.
The provider was deficient in meeting this standard because the was not accompanied by a physician’s statement that he/she does not have communicable tuberculosis and has been under regular care and monitoring for tuberculosis
3) Section 22000(a) Participant Rights
The above section states, “Each participant shall have rights that include, but are not limited to, the following: (7) To be accorded access to his or her file.”
The provider was deficient in meeting this standard because the rights that are provided to participants do not include access to his or her file.
4) Section 22000(b) Participant Rights
The above section states, in part, “Each participant shall review, sign, and be provided at admission, a copy of the participant rights. The program shall place the original signed participant rights document in the participant’s file.”
The provider was deficient in meeting this standard because the participant rights provided was not in accordance with the Alcohol and/or Other Drug Program Certification Standards.
